Community Health and Wellbeing Worker Project – Bringing Brazil to Selby!

Community Health and Wellbeing Workers play an essential role, supporting households to access health services and community support, helping to address rising inequalities in health and social care, and identifying unmet needs in the community.

Each Community Health and Wellbeing Worker has a defined number of households and this project is currently working on streets in Selby West Ward (around Flaxley Road). Workers visit people in their homes to provide health and wellbeing advice and connect them to NHS and community-based services and activities.

Based on an existing health model in Brazil, this exciting project is being delivered by Selby Town Primary Care Network in partnership with Up for Yorkshire, funded by the Integrated Care Board.

Learning Disability advocates share their experience

Selby Town PCN have collaborated with KeyRing North Yorkshire to support some of Selby’s Self Advocates to create an informative video about their experiences at the GP surgery. In the video we meet Tony, Tom, Sam and Joe who worked with KeyRing and Selby Town PCN to create a script and film a video at Beech Tree Surgery. The video explains the importance of reasonable adjustments and the fair treatment of people with a learning disability.
