We currently have three major areas of focus for our work

1. Community engagement

We are building relationships with our communities, by partnering with our local authority, voluntary agencies and others. We want communities to be vibrant and thriving, and have a voice in locality and neighbourhood developments.

We want communities to feel connected and supported and confident in their role in the town. Ultimately, we want to co-design, co-produce and co-deliver locality initiatives with our communities.

2. Population Health and Health Inequalities

We know that adverse health outcomes are unevenly distributed across our communities, and we want to make it our priority to address these so that fewer people suffer from avoidable and unwarranted ill-health.

Working together as a collaborative network with our partners and communities is crucial to this.

3. The Redesign of Primary and Community Care

We recognise that health services are not always configured in ways which help people as well as they should, We want remedy this by working with other service providers to redesign primary and community are services to meet the needs of our populaiton much better.

The current focus for this is on mental health and learning disabilty, urgent care and frailty services.

Current Projects

Community Health and Wellbeing Worker Project – Bringing Brazil to Selby!

Community Health and Wellbeing Workers play an essential role, supporting households to access health services and community support, helping to address rising inequalities in health and social care, and identifying unmet needs in the community.

Each Community Health and Wellbeing Worker has a defined number of households and this project is currently working on streets in Selby West Ward (around Flaxley Road). Workers visit people in their homes to provide health and wellbeing advice and connect them to NHS and community-based services and activities.

Based on an existing health model in Brazil, this exciting project is being delivered by Selby Town Primary Care Network in partnership with Up for Yorkshire, funded by the Integrated Care Board.
