Job summary

We are looking for a frailty nurse to join our innovative Ageing Well Team across the PCN.

This is an exciting opportunity for an enthusiastic, motivated Registered Nurse, working closely with our O/T, physio, frailty assistant and care coordinators to provide proactive care for the moderately frail. This role is integral to the assessment of patient frailty levels and assisting with review of patients to allow the best access to care required for their support. You will be an autonomous practitioner who is adaptable, has a flexible can-do attitude with a passion for excellence and ability to work well both on your own and as part of a team.

This is a community-based role, applicants must have a full driving license and access to a vehicle daily.

Job summary

This is a venture supporting clinicians & allied healthcare professionals across the four practices in Selby Town PCN.

To address the ever-increasing demand for primary care services there is a recognition of the need for better support for clinicians within practices whilst at the same time undertaking work in connection with Population Health Management. Selby Town PCN have a proven track record of developing new roles introduced as part of Primary Care Networks (PCNs) and this role is new to the PCN.
